The Young Adult Patient Navigator will assist with the management, development, operations, and growth of the HomeGoods Young Adult Lounge at Dana-Farber’s Longwood Campus. The Lounge provides a physical meeting space to allow our patients to connect with peers also receiving care at DFCI, as well as an opportunity to be informed of YA resources and research opportunities available at DFCI. The YA Navigator will develop programming to enhance peer-to-peer interactions for young adults using the Lounge during the day. The YA Navigator will also manage and coordinate the off-hours use of the space for YA activities.
The YA Patient Navigator will report to the Program Manager of the Young Adult Program (YAP) and work closely with YAP Director Dr. Cristina Pozo-Kaderman as well as the co-medical directors of the Center for Adolescent and Young Adult Oncology (CAYAO), Drs. Frazier and LaCasce.
The YA Patient Navigator will work primarily on-site at the Dana-Farber Longwood location. This role requires flexibility to work some evening and weekend hours to support events.

Responsibilities Manage day-to-day operations of the Young Adult Lounge Plan and organize programs in the Young Adult LoungeProvide individualized introductions of the YA resources available at DFCI and facilitate referrals. Facilitate peer-to-peer interactions.Assist with other YA educational events, such as annual Young Adult Conference, or support groups.Assist with ongoing program evaluation and outreach. Collaborate and communicate as necessary with medical team to support patient care.Qualifications
Minimum Qualifications:
Bachelor’s degree or higher in a healthcare related fieldPreferred Qualifications:
Prior health education or clinical patient experience is preferredKnowledge, Skills and Abilities Required:
