We have an exciting opportunity for a Young Persons Transitional Worker who has the skills and experience to assess and engage with our service users, ensuring that they access the right intervention at the right time, in a way that best meets their needs.  

 

No two days are the same and we treat every person who comes into our service as an individual, so being flexible, curious and a good listener are key. 

 

Where: Waltham Forest

Full Time Hours: 37.5hrs per week

Full Time Salary: £27,449 - £31,529 dependent on experience, pro rata for part time hours 

Allowance: £2163.27 Outer London Weighting 

Responsibilities

About the role:

 

Strengthening coordination of multiple and simultaneous transitions to adult support (e.g. health, criminal justice, social care, welfare and education).Providing tailored, age-appropriate care & treatment for 18–25-year-olds within the YPSMSTo engage young people in a range of interventions, utilising a young person-centred approach, empowering them to reduce risk, increase resilience and lead a safe, happy and healthy lives.Providing screening, assessment, psychosocial interventions, integrated care planning, post treatmentIdentifying and responding effectively to potential safeguardingReducing drug and alcohol related harm to service users and the widerPromoting carer, service user and communityProviding advocacy for access to partnershipWorking with service users to support positive, holistic outcomes in relation to their health and wellbeing, enabling them to lead safe, healthy, and purposeful lives: reducing risk and increasing resilience.

 

About you:

Knowledge and experience of providing interventions for service users affected by Substance use.Be an excellent communicator, both verbal and written with good IT skills.Proactive and able to work on own initiative with excellent time management and prioritising skills.Have a good working knowledge of mental health interventions, services, and good practice.Be a supportive team player with strong interpersonal skills with the ability to work in partnership with a wide range of professionals, agencies, and internal/external stakeholders.Ability to manage change successfully in a way that prioritises the needs of service users.Understand the importance of information governance processes and commit to follow and apply all necessary safeguards.Seek out learning opportunities to improve and broaden your professional knowledge and skills and to contribute and oversee the learning and development of others.
