Responsibilities
Risk behavior screening and Brief Intervention
Identify patients who are potential PrEP candidatesIdentify patients with high risk HIV exposure in past 72 hours for expedited evaluation for PEPRaise client’s awareness of their risk and motivating behavior changeProgram Intake and Assessment
Introduce program and perform insurance screeningIf uninsured provide benefits navigationIf insured but not a match to clinics’ covered plans or other referral needs provide referral and linkage to primary careCombination Prevention Education
Provide basic PrEP/PEP education pros and cons of PrEP/PEP what it entails other HIV prevention optionsReferral to Services
Assist with referrals as needed to the following: mental health and substance abuse services general social services; Other prevention services including syringe exchange substance abuse counseling and treatmentBenefits Enrollment
Enroll clients in Covered California and assist patients with Medi-cal applicationBenefits Navigation

Assess patient and refer as needed to the following: Appropriate PrEP provider covered by insurance; Create individuals transition plan with patient which includes identifying their PCP or helping them identify a new one in their planAssist patient as needed with scheduling appointments and providing referral details to PCAdvocates for patient needs assures access to services by communicating with AIDS service providers subspecialty providers and the patient regularly to coordinate care monitor service delivery and evaluate service outcomes.Provides clear clinically concise and timely patient documentation and records service deliveries to assist with the collection and monthly report of data required by funding resources. Maintains complete confidentiality of all client and agency-related information.Maintains professional boundaries with all clients of the C.A.R.E. Program & Clinics.Assumes responsibility and accountability for their own professional growth and practice including current knowledge of HIV/AIDS treatment modalities working knowledge of funding sources and services provided community resources and maintaining current licensure/certification and all required in-service education.Demonstrates proficient written documentation skills.Collects documents and reports patient-level data in Casewatch in a timely manner.Demonstrates competency in utilization of computer/database systems including EMR and EPM.Demonstrates knowledge of federal and state HIPAA laws and obtains the appropriate written authorization prior to the use of disclosure of Protected Health Information (PHI).Provide community outreach and education to promote PrEP/PEP services.Other Duties as AssignedQualifications
