We are seeking passionate and reliable Care Coordinators for our Long Island, Suffolk County Care Coordination Program. The Care Coordination Programs services clients with mental disabilities to promotes health and wellness. The role of the Care Coordinator is to provide a person-centered, recovery-focused system of care for individuals.
