(https://hr.uw.edu/wp-content/uploads/sites/3/2018/02/benefits-professional-staff-librarians-academic-staff-20230119\_a11y.pdf) **UW MEDICINE VALUE BASED CARE** has an outstanding opportunity for an **VALUE BASED CARE MANAGER** **Work Schedule** + 100% FTE + HYBRID – PRIMARILY REMOTE – ONSITE AS BUSINESS NEEDS DICTATE **POSITION HIGHLIGHTS** + Improve clinical value, patient outcomes, reducing total cost of care, and promoting health equity + Implement coordinated care strategies, in order to decrease avoidable emergency department visits and hospital re-admissions, ultimately enhancing patient satisfaction **DEPARTMENT DESCRIPTION** The Office of the Chief Population Health Officer (CPHO) under the Chief Medical Officer is charged with system-wide population health and value-based care strategy and integration. **PRIMARY JOB RESPONSIBILITIES** + Direct and facilitate seamless communication and collaboration among multidisciplinary teams across different healthcare entities, including hospitals and clinics + Use data analysis driven methods to identify all value-based patients and intercede with care coordinators across UW Medicine to coordinate care services + Utilize data analytics to identify and stratify high-risk patients, enabling targeted interventions that prevent hospitalizations and emergency department visits and are clearly documented in the EHR + Direct coordination of entity-wide resources to ensure robust transitional care follow up is completed for patients traversing across care settings and ensure care plan adherence to optimize clinical value and reduce risk of readmission + Participate in quality improvement initiatives by analyzing patient outcomes and identifying areas for enhancing care delivery + Report on performance metrics related to value-based care is also essential to track progress and outcome **REQUIRED QUALIFICATIONS** + Bachelor's degree in Nursing, Social Work, or related field + RN, LCSW, CCM or ACM + 5 -6 years of experience in care management, case management, or a related healthcare role.
