VillageCare is looking for self-motivated, passionate RN's for our Per Diem Nurse Assessor role. The Nurse Assessor conducts comprehensive assessments of all aspects of potential new members' condition - clinical, social and environmental to determine the level of services needed to appropriately keep member safe in the community. \n

Responsibilities will include:\n\nUtilize clinical skills to assess and document all aspects of the potential members long-term community based needs.\nProvide community-based training and education for frail population care. \nConduct face to face UAS-NY assessments according to state guidelines, policies, procedures, and protocols.\nGather clinical information from various sources to address all conditions.\nCommunicate with consumers, families, providers, and other parties as needed to complete accurate comprehensive assessment. \nExplain VillageCareMax features and benefits, including explanation of the member's handbook.\nConsult with supervisor and others in overcoming barriers in meeting goals and objectives.\n\n

We would like to speak to those who have a current unrestricted NYS RN license along with 1+ year home care, hospice, or field based case management experience preferred. Proficiency in use of UAS-NY required. Prior MLTC\/ Managed Medicaid\/MAP\/FIDA experience preferred. In home visits required. Bilingual skills required - Spanish, Korean, Russian, Cantonese, and Mandarin\n
