TRIAGE NURSE (RN) - HOME CARE

Summary
Provides clinical support to the clinical services team by assisting in the coordination of the delivery of patient care services. Performs phone triage, data/order entry, patient/staff scheduling, and other clinical tasks related to the provision of patient care.

Job Duties

Triages patients through phone calls utilizing critical thinking skills and sound clinical judgementPerforms data/order entry, patient/staff scheduling, and other clinical tasks related to the provision of patient careProvides hand-off communication to field staff, Clinical Specialist, and/or Patient Care Manager as appropriate regarding triage information, patient status updates, and patients who require a follow up visit or phone call.

Minimum Qualifications
