Overview
We are looking for a Triage Coordinator to join our team. This position will directly report to the Telephonic Triage Manager and is responsible for taking and making calls related to DME, escalating visit needs and scheduling field nurse visits. This role supports the Triage queue and Triage clinicians, as well as manages and arranges care activities on behalf of the Centralized Triage Department – all occurring after-hours/outside of branch business hours (nights and weekends)
The shift is Friday night 5pm-130am and Saturday is 930a-6p and Sunday is 9a-530p est.
Facilitate the appropriate and timely delivery of health care services after hours to improve patient outcomes including but not limited to organizing care and facilitating communication regarding medical needs, visit scheduling, inter-department communications, and DMEUtilize a variety of tools and methods to quickly provide patient visits and education including DME, nurse visits, and leadership escalations outside of branch business hoursAssist in the Centralized Triage process by working closely with the Gentiva Triage Nurses and branch staff to coordinate patient immediate needs after-hours and follow up care About You Associate degree preferredMinimum of 1 year working in a healthcare related field or healthcare call centerINTERNET REQUIREMENT: In Home Wi-Fi Download speed of 50 mb/s and minimum upload speed of 20 mb/s or higher.
