Amergis Healthcare Staffing is partnered with a much-respected facility in Cape Cod, MA who is currently seeking a Home Health Physical Therapist to assist them on a contractual basis. Details of the position include: Position: Home Health Physical Therapist Location: Home visits to patients in the upper, mid, lower, or outer Cape Cod Start Date: ASAP Duration: 13 weeks (with likelihood of extension) Schedule: + 40 hours per week + Monday – Friday + Day shift (5x8’s) + Plan to work 2 weekends during the 13-week assignment + Occasional holidays Experience/Contract Requirements: + Prior home visit experience required + Must be able to evaluate and start PT care + Current Massachusetts PT license + Current BLS certification + EPIC experience preferred + COVID vaccine not required (declination accepted) Job Description: Start of care and case management of a home-based caseload of a variety of patients (up to 6 units each day) Please feel free to contact me with any questions you may have regarding the position.
