I.    Major Responsibilities: 

1.    Survey Administration:
•    Conduct structured patient surveys in a professional, inclusive, and respectful manner.  Explain  the purpose of the survey and ensure that informed consent is obtained before starting.
•    Utilizing interpreter services and other assistance when necessary and to meet the needs of patients.
2.    Data Collection:
•    Accurately document survey responses  , ensuring questions are answered comprehensively.
3.    Data Entry and Management:
•    Enter collected survey responses into a designated database system with precision.
•    Conduct regular quality reviews  on data entries to ensure completeness and accuracy.
•    Ensure all data is securely stored in compliance with organizational and legal standards.
4.    Participant Engagement:
•    Refer participants to additional resources or support services when appropriate.
•    Escalate safety or clinical concerns to supervisors or care teams when needed.
5.    Collaboration and Reporting:
•    Assist with the development and producing standard and customize reports.
•    Provide feedback on challenges or areas for improvement related to accessibility and inclusivity.

Standard Staffing Level Responsibilities:
•    Complies with established departmental policies, procedures, and objectives. 
•    Attends variety of meetings, conferences, seminars as required or directed. 
•    Participates in use of Quality Improvement in daily operations. 
•    Complies with all health and safety regulations and requirements. 
•    Respects diverse views and approaches, demonstrates Standards of Respect, and contributes to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients, and visitors.
•    Maintains, regular, reliable, and predictable attendance. 
•    Performs other similar and related duties as required or directed.

II. Position Qualifications
License/Certification/Education:
Required
•    High school diploma or equivalent .
Experience/Skill:
Required
•    Excellent interpersonal and communication skills, with an ability to connect empathetically with individuals from diverse backgrounds.
•    Sensitivity to mental health challenges.
•    High level of discretion and confidentiality in handling sensitive information.
•    Proficiency in data entry and database management systems, with attention to detail.
•    Compassionate, empathetic, and nonjudgmental attitude.
•    Patience and adaptability to address diverse participant needs effectively.
•    Professionalism and composure in managing sensitive or challenging situations.
Preferred
•    Experience working with individuals affected by substance use disorder.

III. Physical Demands and Environmental Conditions
•    This role will involve working in healthcare facilities.
•    Some travel may be required depending on the program’s scope.
•    Flexible hours may be needed to accommodate participants' availability and preferences.
