Summary:
Under the general supervision of the Manager of Care Navigation or Director of Case Management the Care Navigation Supervisor performs all the functions of a Case Manager Outpatient High Risk Case Manager and Registered Nurse within the Department of Care Coordination. The Supervisor will perform supervisory duties for various system programs for the Care Coordination Department including but not limited to Community Paramedicine Outpatient High Risk Case Management Surgical Services Case Management and Nurse Care Navigators.
For the Community Paramedicine Program the Supervisor reviews patients who have been identified as potential paramedicine patients and follows patients while enrolled in the paramedicine program as appropriate. Also collaborates with outpatient providers and services to ensure adequate patient follow up in the community.
For the Surgical Services Program the Supervisor will interview and obtain home care services for patients throughout the system prior to their surgery.
For all programs the Supervisor will perform supervisory duties including overseeing of employee�s performance (i.e. chart audits) scheduling and escalation of patient barriers and concerns etc. The Supervisor will assist with data collection for each program and follow up with stakeholders as needed.

Performs all duties of the Case Manager and demonstrates expertise in clinical specialty.
Supports all through-put and capacity management activities for the Brown University Health System.
Provides case management consultation within and outside the hospital.
Maintains Paramedicine program data and collaborates with the Brown University Health paramedicine team as well as outpatient agencies providing services.
Coordinates paramedicine services including reviewing appropriateness for referrals and discharges from the program.
Participates in system-wide 24/7 coverage for clinical and/or scheduling issues.
Provides supervisor coverage for case management supervisors as needed.
Assists with Case Management Staff orientation as needed.
Maintains and enhances professional expertise through educational activities.
Adheres to Brown University Health Hospital and departmental policies and procedures to maintain a safe environment of care and compliance with all laws regulations and standards.
Performs other duties as assigned.
Other information:
Licensure as Registered Nurse in the State of Rhode Island by the Rhode Island Board of Nursing or licensure as a Registered Nurse in accordance with the Nurse Licensure Compact agreement of the National Council of State Boards of Nursing.
EXPERIENCE:
Three to five years of clinical case management experience preferred.
Demonstrated knowledge and skills necessary to provide patient care and to develop or administer care management services with consideration of stages of human development and cultural patterns.
Analytical skills necessary to evaluate patients programs and procedures.
Demonstrated ability to exercise independent judgement. Refers specific complex problems to Manager of Case Management or designee where clarification of departmental policies and procedures may be required.
Interpersonal skills necessary to establish and maintain effective working relationships.
WORK ENVIRONMENT AND PHYSICAL REQURIEMENTS
Must have unrestricted driver�s license and reliable transportation. Transports patients as appropriate.
Works in a normal office environment with exposure to patient care.
Must be able to lift and carry ten pounds when transferring materials throughout the facility.
General hospital environment with occasional stressful conditions associated with patient care. Risk of exposure to blood borne pathogens and communicable disease is minimized and controlled by adherence to Hospital Infection Control policy and procedures.
Must be able to make hospital rounds through various patient care areas either by walking or through some other mobile means.
Visual acuity and finger dexterity is needed to review medical records navigate through automated system screens and type on a typical computer terminal keyboard.
Must be able to lift and or carry up to 10 lbs. in order to transport items from one patient care unit to the next.
SUPERVISORY RESPONSIBILITY:
Supervises up to 6 FTEs
