OVERVIEW:
Under the supervision of Shelter Director, the Program Coordinator will coordinate support services and collaborate with community partners to assist clients in the Short-Term Post-Hospitalization and Housing Program by building relationships with all CalAIM entities, other needed community service organizations, and other necessary medical providers.
DUTIES AND RESPONSIBILITIES:
Build relationships with all CalAIM entities, hospitals, and rehabilitation facilities Ability to build strong networks within the community and to build new relationships and partnerships for the purpose of strengthening the program and increasing service access for program recipients. Supervision of program staff Liaise with medical on-call Report to CalAIM as required Maintain client case records, files, and required forms Establish and maintain a working relationship with all referring agencies Monitor contractual requirements as related to the program Other related duties as requiredEDUCATION AND EXPERIENCE:
High school diploma or equivalent (Required) Associate degree (Preferred) Experience with Trauma-Informed Care Experience supervising a team (2 years preferred) Experience in health care or community-based services (2 years) (Required) Experience training in substance abuse and/or behavioral health Experience with data sets and creating effective presentations Experience with grant programming and/or budgeting (preferred) Experience in data entrySKILLS AND ABILITIES:
Knowledge of HIPAA regulations and confidentiality requirements Strong quantitative, analytical, and technical skills relating to data management, analysis, and presentation Knowledge of HMIS, WellSky, and other software programsPHYSICAL REQUIREMENTS:
