Senior Information and Referral Specialist – Family Support Services Help Others to ASPIRE HIGHER! We have a great full-time opportunity for a positive, gregarious employee to join our Family Support Services team. In this role you will perform the information and referral function and execute the non-clinical requirements of the OPWDD Family Training Grant. Provide coordination of services to Plan of Care individuals. Coordinate the intake process for the STRIVE, Respite, and PWW Programs. Essential Functions: + Provide Information and Referral to community and agency personnel. + Conduct information/life assistance planning sessions with individuals or families. + Provide Information and Referral to agency personnel, families, individuals, and to external agencies. + Attend all OPWDD Family Support Services Network quarterly meetings. + Assist families applying for family support services and grants, as applicable. + Oversee the intake process for all Respite, STRIVE, and PWW Programs. Determine the appropriateness of referral, track acquisition, and review of supporting documentation; assess and recommend appropriate programs; monitor screening and attend applicable screening/ intake meetings. + Liaison to other departments/ programs as related to STRIVE, Respite, and PWW placement. Representation on the following committees- Residential Admissions/ Movement, Day Services Census + Provide vacancy and census information for PWW to the Day Services Intake Coordinator. + Review and document all applications received for ACLD services. + Complete home visits and meeting for the individuals on the grant, as required. + Complete all required documentation for Family Member Training grant/services. + Provide short-term care coordination, as applicable. + Review rights and responsibilities and grievance procedures with people we support and their families. + Review financial records of people we support. + Attend all mandatory departmental meetings. We require: + Bachelor’s Degree in Special Education, Health Services or Social Services field. Master’s Degree preferred. + Minimum 7 years experience working with individuals diagnosed with developmental disabilities. + Extensive knowledge of developmental disabilities, the OPWDD delivery system, and an understanding of family dynamics. + Clean and valid driver’s license and the use of your own vehicle on a daily basis. + Must be flexible to represent ACLD at evening/weekend transition fairs.
