POSITION SUMMARY:
Plans and implements prescribed feeding/communication therapy treatment programs for persons who receive services to maintain and/or restore function secondary to acute or chronic disability; conducts annual screens, assessments, and daily documentation if warranted.
POSITION RESPONSIBILITIES:
1. Reviews MD’s referral and persons receiving services medical record prior to determining speech/language therapy services. Refers to specialty clinics as needed (ACC, dysphagia). Maintains active/up to date MD prescriptions as outlined by NYS Department of Health (DOH) and Office for People with Developmental Disabilities (OPWDD), depending upon work assignment.
2. Evaluates and assesses the persons receiving services’ needs and level of functioning in the areas of: speech, language, cognitive & social aspects of communication, voice, augmentative and/or alternative communication (ACC), and eating /management of diet (dysphagia). Evaluate person receiving services within facility-specified time frame.
3. Confers with physicians and other health care practitioners to obtain additional information. Integrates input from persons receiving services, family, and direct care staff in treatment assessment and recommendations.
4. Devises treatment plans based on the assessment; administer treatment involving the application of communication and/or feeding aid modalities: augmentative/alternative communication devices, basic ASL, suggest adaptive feeding equipment; instruct, motivate, & assist persons receiving services in program to enhance communication skills.
5. Writes specific, objective, and measurable documentation which is outcome oriented and details the persons receiving services’ needs, potential to benefit from treatment and progress achieved to date. All documentation must be completed through EMR within the time frame established by the Healthcare Center or Article 16. Record persons receiving services’ treatment, response, progress and outcome/supportive programs.
6. Consults with supervisor when appropriate to implement recommendations.
7. Provide training to persons receiving services, families, direct care and/or outside agencies if warranted.
8. Communicates effectively with discharge planning from rehabilitative programs.
9. Participates and attends care plan/team meetings for all persons receiving Speech-Language Therapy services.
10. All documentation must be completed through EMR within the time frame established by the Healthcare Center or Article 16. Perform oversight in the Healthcare Center, Article 16, day program, and residential settings as applicable.
11. Performs all other duties as assigned.
POSITION QUALIFICATIONS:
1. Graduate of an accredited Speech-Language Pathology school; Master’s Degree, ASHA Certificate of Clinical Competence, and New York State license.
2. Knowledge in Speech-Language Pathology evaluation and treatment; understand the principles of modalities employed in SLP; knowledge in the use of augmentative communication devices, therapeutic feeding techniques, speech and language intervention strategies. Must have good written-verbal communication skills and be computer proficient.
3. Must be able to travel as assigned to other Healthcare Centers, Article 16, day programs, or residential sites, if warranted.
REQUIREMENTS:
1. Maintains current licensure and certification.
2. Performs all job responsibilities in full compliance with all applicable laws, rules, regulations, policies and procedures. Adheres to the agency’s corporate compliance program, including the code of business conduct, the NYS and federal false claims acts, whistleblower protections and mandatory reporting. Completes corporate compliance training at least annually. Fully cooperates in all corporate compliance investigations and reviews.
