Job Description:
The Social Worker (MSW) is a non-clinical social work position responsible for discharge planning, case
management, and psychosocial support of patients and their families within the hospital setting. This
involves completing psychosocial assessments and interventions to assist patients in coping with their
hospitalization, illness, diagnosis, and/or life situation and provide support and education to families on how to support the patient after hospitalization. The Social Worker also provides advice or guidance for any other non-medical problem or barriers to care patients may have that may hinder their rate of recovery or delay discharge and serves as a resource for continuity of care options after discharge.
Requirements:
Master's degree of Social Work from a school of social work accredited by the Council on Social Work Education
KY license as a Certified Social Worker (CSW) preferred upon hire. Required within 2 years.
Preferred Qualifications:
Two years of social work experience preferred.
