Palliative Care Partners has an exciting opportunity for a Part-time inpatient social worker looking to serve as an expert caring for patients with serious illness as well as play an active role on the palliative care team.  The primary role of the palliative social worker is to identify patient and family psychosocial needs and priorities of care through assessments, care planning, clinical interventions, education, and evaluation of outcomes.

What you will do:

The social worker acts as a facilitator for patient-family meetings for goal setting and advanced care planning, coordinates educational and outreach services relevant to the social service-palliative care interface, and serves as liaison between system wide inpatient and outpatient palliative care teams, hospice, and other community services.

Responsibilities:

In this role, the social worker will participate in daily interdisciplinary rounds to triage & discuss complex cases, use clinical judgment and discretion in determining which patients need specialized palliative care social work support, and the frequency and duration of visits. They will also conduct trauma-informed psychosocial-spiritual assessments with hospitalized patients and their supports and implement therapeutic interventions to aid in symptom alleviation and enhance coping. They will attend to existential distress and anticipatory grief that may accompany advanced illness and/or the dying process, as well as engage patients & visitors in advance care planning.

The palliative social worker is an integral part of the Interdisciplinary team developing and coordinating collaborative care plans with Physicians, Nurse Practitioners, and other members of the care team.

What you will need:

Masters in Social Work/LMSW/LCSW or LMHC required.Three years previous clinical experience in Medical, Psychiatric or Health Care Setting required. Two years post MSW preferred. In depth knowledge of Healthcare, Healthcare Delivery: including but not limited to Acute Care, Continuing Care, and Ambulatory Care Services. Knowledge of IHANY, ACO's, MSSP clientele, Health Literacy, Community Resources, at risk populations and Advanced Payment Models in an effort to prevent sub-optimal use of Healthcare and in alignment with Trinity's Priority Triple Aim.Knowledge of community resources required.Proven leadership ability as a team leader of groups, committees, and or project development.Some supervisory experience preferred.

Pay Range: $31.50-$45.23

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
