Job Overview:

Central Jersey Behavioral Health is seeking a per diem MSW/LSW to assist patients and their families to obtain the maximum benefits of available health care services through psychosocial assessment, counseling, discharge planning and referral services. The Social Worker provides emotional support to patients, either as a group or individual, through group process and psychotherapy.

Qualifications:

Required:

Master s Degree in Social Work from an accredited college or university

Preferred:

Licensed Clinical Alcohol and Drug Counselor (LCADC)

Certifications and Licenses Required:

Licensed by the New Jersey Board of Social Workers

Scheduling Requirements:

Shift working, 8:30am - 5:00pm Per Diem

Essential Functions:

Provides family counseling session whenever possible or clinically indicatedParticipates in monthly Social Work meeting with the Director of Inpatient Social ServicesParticipates in and assists in the development of the interdisciplinary treatment planning processAdheres to Attendance and Reliability Standards. Gives proper notification for absence or tardiness as per policyImplements the policies and procedures of the Behavioral Health facility. Performs interventions in accordance with current policy and procedure, safety guidelines and infection control principlesEstablishes rapport with family or significant other whenever possible or clinically indicated. Involves patient s family or significant other in formulation of the treatment plan and establishment of effective discharge plans and aftercare whenever possible or clinically indicatedFacilitates access to community agencies by providing referrals and information to patients, their significant others, community agencies and other community members advocating for patients when necessaryShort term counseling; to assist patient in identifying adaptive means of coping through individual and group therapyAssesses patient's psychological, social, medical and substance abuse issues through direct patient interview or interview/ contact with patient s significant others with respect to their current needs to establish appropriate post-program care plans and any services that may be necessary while patient is in treatmentReviews discharge planning options and evaluates alternatives with patient and their significant others to enhance their acceptance of plans and servicesIntervenes with patients and their significant others to enhance their ability to cope with mental illness, hospitalization; post-hospital care needs, and lifestyle changesPerforms other duties as directed by Department Head/Supervisor
