**Responsibilities**
In keeping with the Marian Regional Medical Center healthcare philosophy the Social Worker is responsible for providing direct casework services to clients patients and families and for collaboration with appropriate healthcare workers and community agencies. Some of the services may include Family Caregiver program Transitional Care Center (TCC) and CalAim - Enhanced Care Management (ECM) which are all services within the Population Health Department and report to the Sr Director- Administrative Services. The MSW will engage with patients both in person in a field-based setting hospital clinic home and/or on the phone in a manner that utilizes evidence-based approaches that promote collaboration between the patient and his or her mental and/or physical health goals.
**Qualifications**
+ Two years social work experience in a healthcare setting.
+ Familiarity with Managed Care and discharge planning.
+ Strong diagnostic skills and experience working with homeless and/or SMI populations are also desirable for the role.
+ Master’s Degree - Completed Masters Degree in Social Work.
+ AUTO:CA AUTO-INS; BLS-CPR
**Pay Range**
$35.28 - $51.16 /hour
