Responsible for counseling, assessing, and assisting geriatric patients and their caregivers. 


Principal Accountabilities:


1. Provides psychosocial assessments to determine patient needs, makes appropriate referrals, provides counseling and supportive services to patients/families
2. Collaborate with clinicians on geriatric plans of care

3. maintains and utilizes a current knowledge base of community resources, agencies, and refers patients/caregivers

4. provide assistance with end of life and bereavement

5. educate, assist and intervene with patients and families on legal options such as advance directives

6. Utilize the EMR to document assessments, progress notes, and patient interactions 

7. Complete appropriate forms to facilitate referrals 

8. utilize AHS PRIDE values during patient, caregiver, and team interactions

9. Complete interventions to promote continuity of care after patient discharge from hospital/facility
10. Other relevant duties as assigned
