Responsible for counseling, assessing, and assisting individuals/families or groups who are affected by illnesses or injuries, including rape, physical abuse, cancer, AIDS, and Alzheimer's, that may impair their ability to function normally.
Principal Accountabilities:
1. Provides psychosocial assessments to determine patient needs, makes appropriate referrals, provides counseling and supportive services to patients/families, makes referrals to the Division of Child Protection and Permanency (DCP&P)
2. Identifies and maintains funding sources which supports the Family Counseling & Guidance Program.
3. Maintains statistical information and analysis of data, develops outcome and measures for program evaluation, program development and facilitation of program.
4. Other relevant duties as assigned
