Essential Functions/Responsibilities of Position: + Home visits with home care patients, providing disease counseling, end of life discussions with goals of care, community resource referrals, assistance with applications to community resources, Bi-lingual a plus Qualifications: Minimum Education: + Master's in Social Work from an accredited school of social work required License/Certifications: + NJ Social Work License LSW or LCSW required Relevant Experience: + Disease counseling and community resources
