Job Overview:

The Licensed Clinical Social Worker (LSW/LCSW) provides initial intake assessments, and individual and group counseling services to adults with Serious Mental Illness SMI, including those with a co-occurring substance use diagnosis in our Adult Outpatient department.

The LSW/LCSW participates in planning conferences, maintains up-to-date and complete records of patient information according to established standards, is a member of the multidisciplinary team, creates, reviews, and updates treatment and discharge plans, run groups and communicates with outside providers and referral sources.

Qualifications:

Required:

Master s degree in Social Work or related fieldNJ LSW or LCSWAt least one year experience working in a substance-abuse setting

Preferred:

LCADC

Certifications and Licenses Required:

Current NJ LSW or LCSW license

Essential Functions:

Interprets the nature of illness and prescribed course of treatment to patients and familiesCommunicates family issues to other staff membersParticipates in planning conferencesIdentifies substance abuse/use issues and incorporates into tx plan identifying triggers, support mechanisms and 12-step participation if appropriateMentors and clinically supervises LSWs and students as per license requirements and field instructions as directedMaintains up to date and complete records of patient information according to established standardsDocuments progress notes within 24 hours of DOSUnderstands the payor identification process and ensures that services delivered are accurately authorizedAs a member of the multidisciplinary team, assists in evening and updating treatment planInvolves families/referral source(s) in all treatment endeavors unless contraindicatedAdheres to department s practice guidelinesMaintains current knowledge of professional practice standardsMaintains current knowledge of relevant addictions tx and researchPerforms random UDS on patientsIn collaboration with treatment team, implements the discharge planCollaborates with other hospital programs and community agencies to promote coordination and continuity of patient and family carePerforms the psychosocial assessment which includes family dynamics and treatment needs according to specified outpatient/inpatient standardsDesigns and implements treatment plans, including individual, group and/or family psychotherapy and evaluates and documents patients' outcomes according to departmental policy and proceduresConducts individual, family, and group treatment according to established standards/practice guidelinesRecognizes the need for and implements a strength based framework in treatment planningCollects and utilizes outcome data to support treatment planning/modificationsReviews assessment data including identified medical needs and makes appropriate and timely referral
