Discover a rewarding career at AJH, where you can make a meaningful impact within a close-knit community while advancing your professional growth within a thriving healthcare system._ **_Are you ready to become a part of our dynamic Case Management Team?_** Responsibilities include but are not limited to: + Appropriately identifies and assesses all new admissions for clinical social work intervention based on interdisciplinary case referral and case management high risk criteria. + Develops an individual plan of care based on the psych/social needs of the patient and develops a realistic discharge plan in collaboration with the patient/family/health care team. + Implements a plan of care based on the needs of the patient, providing ongoing support and resources to patients and families. + Effectively communicates with the interdisciplinary health care team discharge needs and obstacles affecting treatment and discharge disposition. Makes referrals to appropriate agencies based on patient need and sets up family/resource meetings. + Prioritizes and manages daily case load based on patient and hospital need. + Ensures that all patients are aware and in agreement with their individual discharge plan, reassessing and updating plans of care to meet patient and family needs, while advocating and respecting patient rights. + Completes accurate and legible documentation, follows mandatory documentation guidelines. + Maintains professional accountability. Maintains customer focus and confidentiality. Participates in hospital/unit-based committees, adheres to hospital and departmental policies, participates in QI processes. Manages time efficiently. Provides support and consultation to other members of the health care team around psychological, social and management issues of patients. + Sensitive to the needs, feelings and capabilities of others. Approaches others in a non- threatening and pleasant manner, shows tact and diplomacy. **Scheduled Hours:** 40 hr/wk **Work Shift:** Day 8 am - 430pm **Education:** MSW or BA in related field with a minimum of 3500 hours of supervised work exp. in social work services. **Required Experience:** Experience in crisis intervention desired. Clinical Social Worker experience, minimum of one year in a hospital setting preferred or related experience. **Required Licensures/Certifications:** Commonwealth of Massachusetts Clinical Social Worker Licensed required.
