Job Summary The Social Worker helps patients and families adjust to temporary or long term life-style changes due to illness and related complexities that impact their social and / or emotional needs. Helps to plan and coordinate programs and activities in support of the patient's total well-being including counseling and coordination of community, social and hospital services.
