Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations._ **Responsibilities:** Provides professional social work services to patients and their families; collaborates with hospital staff and community social services resources relative to the psycho-social problems of patients; participates in formulating procedures and policies of the Division of Social Service; and participates in educational and research programs, including the orientation and training of new staff and students. Responsibilities GENERAL PURPOSE: The social worker assigned to Strong Ties Clinic is responsible for providing psychosocial assessment and social work treatment to individuals ages 18 and older who are severely and persistently mentally ill. Services are provided through participation on an interdisciplinary team designed to provide comprehensive community based psychiatric care. The social worker functions as a primary therapist for assigned patients and provides direct clinical service including individual and family therapy, group therapy, crisis intervention, and community outreach. RESPONSIBILITIES: - Provides direct clinical services to assigned patients, which may involve individual, family, and/or group therapy, crisis intervention, and diagnostic evaluations. - Functions collaboratively and consultatively as a member of the interdisciplinary treatment team. Ensures the development and implementation of comprehensive and individualized treatment plans, to include periodic case review. Requests and coordinates psychiatric consultation when indicated. - Collaborates with family, social supports, community agencies (past and present) to ensure appropriate and effective care. Coordinates referrals and facilitates linkage to other appropriate agencies/supports. - Documents interventions, both direct and indirect, in the patient's medical record in compliance with the Division, Department, and Hospital policy. - Attends Social Work Division, Social Work in Psychiatry and Program staff meetings. Participates in activities of the Social Work and Psychiatry Division Psychiatry Division including, but not limited to, ensuring policies and procedures are implemented with regard to professional practice, discharge planning and patient care, timely completion of time reporting and other Hospital or Division forms; Hospital and Division mandatory in-service training programs and UHS reviews. Participates in routine committee process of the Social Work Division, and facilitates Division and Hospital quality assurance and staff development activities. Subscribes to the departmental goal of creating a culturally competent environment by treating patient, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences. Participates in available cultural competency events and training appropriate to job duties. - Develops and leads groups for patients (and possibly family members or their support network) which focuses on issues and needs of the severely and persistently mentally ill population - Provides back-up coverage for patient care, as appropriate and requested. - Other duties as assigned QUALIFICATIONS: - Master's Degree in Social Work from an accredited program (CSWE) required. - Prior experience working with individuals with chronic mental illness and/or community mental health preferred. - Knowledge of mental illness preferred. - Ability to engage in person centered treatment preferred. - Knowledge/skills in various therapeutic interventions including CBT, DBT, motivational interviewing preferred. - Knowledge of risk evaluation and mitigation preferred. - Master's degree from an accredited school of social work required. Licensure as a social worker, as defined by the State Board for Social Work, State Education Department, Division of Professional Licensing Services is required. If licensure has not been obtained upon employment, the individual has 18 months within which to obtain it, unless duties will be performed under OMH then minimum of a limited permit (not to exceed 1 year) or NYS Licensure will be required upon employment start.
