SUMMARY
Responsible for performing and/or supervision of the daily business office functions and operations of non-provider staff in conjunction with manager.

ATTRIBUTES
BSW required
Two years of medical office experience with computer systems.
Computer experience needed with Word, Excel, Epic.
Must have excellent keyboard skills and be familiar with the Windows environment.
RESPONSIBILITIES
Documentation. Manages referrals related to patients at risk with psychosocial needs and determines appropriate interventions and strategies to meet those needs. Determines patient needs through interdisciplinary rounds, chart review, patient/family interviews and team conferences. Documents social work intervention.
Assessment. Assesses patient needs and determines mode of intervention. Coordinates assessments and develops care plans in accordance with accepted social work policy as documented in chart. Coordinates multidisciplinary and agency case conferences as needed.
Planning. Implements plan of intervention preparatory to discharge or initiating continued care plan in compliance with departmental and governmental regulations
