We are seeking a Social Worker to provide support and services to residents within the healthcare and villages as required. The Social Worker have to do psychosocial assessments, advocate for the residents’ best interest and provide emotional support

Specific duties include but is not limited to the following:

Provide emotional support and counselling to residents and/or their family membersGuide residents through the transition into care or changes in health statusFacilitate care decision-making when family members are not availableHelp navigate and de-escalate challenging family dynamicsCoordinate and support the process of curatorship applicationsImplementation of the Older Persons Act 13 of 2006Present therapeutic group sessions to residents and or their family members.Participating in monthly Multi Disciplinary Team meetingsProviding training to various members of staff based on need and requestWriting health educational/promotional material for newsletters to families and residentsResponsible for meeting defined key performance indicators (KPIs) Leave cover for critical positions such as the Discharge Planner/ Rehabilitation Coordinator within our Sub-AcutePost discharge follow up phone calls with patients 

Requirements:

Qualifications 

Bachelor's Degree in Social Work 

Experience and Knowledge

Relevant experience in a similar roleTraining and experience in counsellingExperience in investigative social workTraining in bereavement, palliative care and dementia care will be beneficialExperience working with the Mental Healthcare Act will be beneficialKnowledge pertaining to external private and government frail cares and home care services will be beneficialKnowledge of the Older Person ActPrevious experience with curatorship applications would be beneficial
