DESCRIPTION OF WORK In this role, you will work closely with youths, families, and community resources to ensure a smooth transition for individuals leaving the Loysville Youth Development Center. You will be involved in coordinating and implementing transitional services, creating discharge plans, establishing a network of community-based resources, and conducting follow-up after the individual has left the facility. Work Schedule and Additional Information: + Full-time employment, 37.5 hours per week + Work hours are 9:00 AM to 5:00 PM, Monday - Friday, with a 30-minute lunch.
