Social Worker | Rice Memorial Hospital

As an integral part of Rice Memorial Hospital Care Management team, the Social Worker focus is to provide professional, medical social work services for hospital patients/families in assigned inpatient and outpatient units/departments. The individual in this position practices medical social work focusing on patient’s/family’s psychosocial needs through assessment, discharge planning, patient/family education regarding available resources, advocacy, support and linkage to community referrals. You will represent Rice Hospital on community boards, task forces and other activities assigned by the director, and be responsible for program development, evaluation and improvement in assigned clinical area.

Qualifications:

Minimum of Bachelor's degree in Social Work with 2 years medical experience required. Master's degree in Social Work preferred.  Licensed by the State of Minnesota. Previous experience in a medically related setting required.

Additional Qualifications:

Knowledge of the psychosocial aspects of illness/disability/ hospitalization. Ability to assess needs of patients and families. Knowledge of community resources and how to access them. Requires knowledge and ability to utilize skills in supportive counseling and support group facilitation. Knowledge of crisis intervention, vulnerable adult and abuse regulations. Requires excellent verbal and written communication skills.

 

To be eligible for the hiring bonus you must be an external candidate who is hired into an eligible full time or part time benefited position. Previous CentraCare employees qualify if they have not worked for CentraCare for at least 9 months at the date of hire.

Relocation assistance requirements: Must accept a position that offers benefits. Must currently reside more than 50 miles away from Willmar, Minnesota and relocate to within 30 miles any time before and no later than 120 days after you start work. May not have worked for CentraCare in the past nine months.
