Full-Time Social Work Associate Reports To: Social Work Manager Queens/Manhattan, New York United States Responsibilities : + Provide concrete case management, follow-up and referral service + Provide entitlements/benefits eligibility counseling and assist with application + Complete psychosocial assessment + Formulate and implement the intervention plan for patients + Assist Clinical Social Workers in providing case management service as needed + Attend case conference, agency meetings and staff meetings + Participate in community outreach and marketing activities + Perform other duties as assigned Minimum Qualifications : + Bachelor's Degree in Social Science, Social Work, and Psychology, Sociology or other related field + At least one year of working experience in social services, health care agency or related field + Bilingual in English/Chinese-Mandarin or English/Spanish Compensation and Benefits : + Salary: $50,000.00 - 55,000.00 per year + Medical insurance + Employer-paid dental, vision, disability and life insurance + 403(b) retirement savings plan + Flexible spending accounts for transit, parking, health and dependent care + Paid time off (28 days for the first year of employment) + Employee Assistance Program + Tuition reimbursement for eligible employees, and + The Health Center is qualified for Public Service Loan Forgiveness (PSLF) program
