Overview

The Social Work Assistant provides social work services to patients in their place of residence, in accordance
with the physician orders and plan of care under the supervision of a qualified Social Worker and Clinical
Manager or designee.

Responsibilities Understands and adheres to established company policies and proceduresParticipates in the coordination of company services with community resources and services, serving as liaison between patients or family/caregivers and community resourcesMaintains collaborative relationship with organization personnel to support patient careObserves confidentiality and safeguard patient informationServes as a resource for employees, patients, physicians, and other allied health providersDevelops a cooperative relationship and communicates effectively and professionally with healthcare providersMaintains current knowledge of Medicare Conditions of Participation for social work servicesParticipates in in-services and/or continuing education programsParticipates in patient care conferencesMaintains contact with community support groups and provide professional expertisePlans and coordinate all social services within the companyDocuments all patient/family services provided as required by company policyAssists physician and/or other care team members in understanding the significant social and emotional factors related to health problemsParticipates in the development of the plan of carePrepares clinical and progress notes in accordance with company policyParticipates with patient discharge planningOther duties as assigned and within scope of practice Qualifications Baccalaureate degree in Social Work, Psychology, Sociology, or other field related to Social WorkOne year of social work experience in a healthcare setting, three to five years preferredOne year in home health care preferredValid license from the state practicing in when required by state lawCurrent CPR certificationBasic clinical skillsProfessional appearance and demeanorGeneral knowledge of computers and the ability to learn industry specific software applicationsExcellent oral, written, and interpersonal communication skillsExcellent organizational and time management skills with a keen attention to detailsAbility to work effectively in a team environmentAbility to develop critical thinking skills, research situations, solve complex problems and deal with a variety of issuesAbility to learn and complete various educational goals during orientation About our Line of Business Adoration Home Health and Hospice provides quality and compassionate services in the comfort of home, providing support for patients, families and caregivers in their time of need.  Adoration was formed to fill the need for a loving, community-focused, caring organization.  We empower patients to live with dignity, find a sense of fulfillment, and celebrate with their families a life well-lived.  Our employees and caregivers are proud to be a part of the Adoration team and the mission of our company.  For more information about Adoration, please visit www.AdorationHealth.com.  Follow us on Facebook and LinkedIn.
