Position Purpose:
(InThe Social Services Technician collaborates with other members of the multidisciplinary team in caring for the psychosocial, financial, emotional, environmental factors as they relate to patient health needs; acts in concert with the social services, population care management colleagues, nursing, medical, and other support staff in providing assistance and supportive measures to patients and families for care coordination and care management – specifically focused on connecting patients with community resources and helping patients to be successful in removing socio-economical barriers; participates in ongoing continuous quality improvement processes to improve patient care.

(Utilizes population care management tools and processes to identify, engage, and coordinate care coordination and care management activities for patients. (skills)
Utilizes knowledge of community resources to assist patients in removing or reducing socio-economical barriers. (skills)
Assists in assessing patients and identifying psychological, social, spiritual, environmental and educational needs. (skills)
Establishes a trusting and supportive relationship with patient to support a safe and respectful environment (skills)

Minimum Qualifications:
Education
Bachelor of Science or Arts degree with a major in social services, psychology, sociology, human relations, health related field required.
Experience
Previous experience working in connecting individuals to community resources is required.
special Skills, Competencies, and Aptitudes
Must provide proof of Basic Life Support (BLS Healthcare Provider, American Heart Association; or American Red Cross, Professional Rescuer-LIVE class only) via current card prior to employment or within 60 days of hire and maintains current status.
Completes Mandatory Reporting: Child and Dependent Adult Abuse requirement within 6 months of hire and renews this requirement every 5 years.
