Social Services Supervisor

Nexus-PATH is looking to add a Social Services Supervisor to our Independent Living Program Team!

Position Summary: Our role will provide oversight of Nexus-PATH’s Chaffee Independent Living program, staff management, and leadership, to ensure operational efficiency of the Program within all applicable rules and standards of the organization, as well as external regulating agencies.

Primary responsibilities:

Provides comprehensive coordination of all tasks required to meet the needs and best interest of service recipients consistent with DHS, COA, Nexus-PATH mission, values, policies, ethics, and practice.

Identify youth who are age 16 and older, and who are likely to remain in care until the age of 18. Administer assessments and develop plans. Verify eligibility for Chafee Funds. Assure that Chafee expenditures are allowable. Prepare an annual plan and budget for region’s Chafee Program. Assist eligible young adults with the ETV process. Utilize the wraparound process in developing, implementing, and monitoring the Independent Living plan for the service recipient. Identify and document available match dollars. Facilitate monthly youth group meetings on independent living. Assure that supplementation of funds does not occur.

Ensure that documentation is thorough and timely as required by standards.

Provide documentation of flex fund expenditures on a monthly basis. Conduct and document diligent efforts to obtain follow-up data on youth who have exited the program. Respond to requests for data from the state coordinator. Provide quarterly and year end reports to state coordinator.

Promote the needs of service recipients to ensure successful outcomes.

Work to understand and adjust to cultural differences relating to client assessment, communication, and relationships; advocate for foster/former foster youth; encourage youth to be self-advocates and leaders. Adjust and adapt to the changing work needs and demands; maintain a flexible schedule to meet the needs of program recipients and clients. Advocate legal rights for service recipients. Advocate for resources to assist service recipients to successfully achieve Independent Living goals. Educate the community about the needs of service recipients. Advocate for the development and maintenance of funding or services to meet youth and family needs at local, state, and national levels; seeks creative ways to problem solve and close opportunity gaps. Develop and distribute written materials to eligible participants; demonstrate exceptional written and verbal communication by articulating oneself clearly, respectfully, and professionally with all audiences.

Represent Nexus-PATH’s mission and values with integrity in all interactions.

Develop a good working relationship with eligible youth and young adults, their caregivers, custodians, informal supports, and social workers, using a positive youth development and wraparound approach. Include input from foster/former foster youth in program design and implementation. Partner with the broader community to provide a supportive learning environment for foster/former foster youth. Utilize feedback to improve Nexus-PATH practices and services.

Participate in and support the ongoing selection, evaluation, supervision, and development of staff.

Participate in recruiting, interviewing, and selecting qualified Independent Living staff, consistent with corporate policies and guidelines. Supervise assigned staff, monitor work progress, establish performance standards, and ensure that all performance evaluations are completed in accordance with Nexus standards. Provide professional development supervision, consultation, and assistance on an ongoing basis to ensure appropriate coaching and development of staff. Provides ongoing case management support, debriefing and monthly case consultations with SIL Coordinators. Develop collaborative and cohesive teams through demonstrated leadership, problem-solving, remaining approachable, and communicating the Nexus mission, values, and shared goals and objectives. Encourage a climate of growth through planned development activities such as direct performance feedback and coaching, job rotation and shadowing, and continuing education.

Ensure confidentiality regarding clients’ medical backgrounds and treatment issues per HIPPA guidelines.

Required Education and Licensure:

Masters or BA degree in social work, psychology, vocational rehabilitation, education, child development, or other behavioral field. 2+ more years of experience working with youth in foster care and/or behaviorally challenged youth, and/or emotionally disturbed youth, and/or youth involved in the juvenile justice system, and/or youth within the educational setting. Proof of valid driver’s license, auto insurance and access to vehicle capable of safely transporting clients. Successful completion of background checks and all applicable health requirements. Must be 21 years of age or older. Valid driver’s license required. Must meet state regulating agency and Home Office driving requirements.

 Nice to Have: 

 3+ years’ experience working with youth in foster care and/or behaviorally challenged youth and/or emotionally disturbed youth. 2+ years’ experience in a supervisory role.

Travel:

Must be willing and able to travel within the community, city, or state level via air and ground transportation.  Use of a personal vehicle for regular local business travel will be required to support the clients and agency business offices or company-sponsored events may be required.
