Site Service Coordinator

Our SeaView Adult Day Health Program located in Mashpee, Ma is seeking a part time Site Service Coordinator (30 hours per week). The hours are day shift Monday through Friday 8:30am - 2:30pm.  
The Site Service Coordinator is directly responsible for the assisting the Program Coordinator with management of the day-to-day operations of the program, monitoring mental health status of participants, and providing case management services.

Duties and Responsibilities:

Assist in oversight of the daily function of the program to include planning, scheduling, coordinating and implementation of appropriate activities, safety and emergency evacuations plans.Assist in oversight of the implementation of new ideas and activities to increase and ensure program growth and quality services to the participants.Support and train on-site staff in conjunction with the standards and requirements established by the Department of Public Health regulations for ADH services as well as Council on Accreditation standards.Assist with the role of coordinator in the referral, intake, admission and discharge processes.Coordinate and serve as liaison with individual participant caseworkers, site directors, counseling therapist, medical staff and participants’ family.Assist in the recruiting, training and hiring of staff including volunteers and interns as requested by Program Coordinator.Respond to the reporting requirements of the appropriate regulatory agencies.Assist in the fiscal administration, including billing, budget preparation and required financial reports.Direct the development and implementation of public relations projects for the program.Provide case management services.Write progress notes quarterly.Participate in the quarterly review of each participant’s care plan as well as assisting with the development of care plans.Attend monthly supervisor meetings with the Program Coordinator (Bi-weekly with LICSW if not licensed).Develop and lead groups for the participant.Assist Program Coordinator with record reviews, documentation, compliance, and implementation of policy and procedure.Inform participants and their families/caregivers of available community services and make referrals as indications warrant.Assist, as necessary, in the delivery of other required program services.Education and Experience:Bachelors Degree in health or human services or a related field required. One (1) year of recent experience working with disabled/elderly adults in a professional capacity is required. Significant work experience with the elderly and/or physically/mentally challenged adults in a community setting preferred.
