The Single Point of Access (SPOA) Specialist is responsible for facilitating referral processing by retrieving, entering, triaging, tracking, and routing referrals for adult-serving systems in the community. Conducting outreach to designated community agencies to provide information about referred individuals to OMH specific housing, forensic case management, health home plus services and other appropriate services available in the community in an effort to increase access and minimize barriers for identified individuals in need of selected services. CCSI is partnering with the Onondaga County Department of Children and Family Servicesto hire aSingle Point of Access (SPOA) Specialist.This position is employed by CCSI and working on behalf of the Onondaga County Department of Children and Family Services In this role, you will be + Serving as the contact for high need individuals in the community and a liaison to referral sources such as the local police departments, hospitals, and other community service providers. + Partnering with individuals in need to complete an assessment of needs, explore supports, assist with problem solving and identifying long and short-term solutions, complete appropriate SPOA referrals, and consent to obtain services and community supports. + Establishing and maintaining positive relationships and open communication with referred individuals, referral sources, governmental units, and community organizations. + Providing education and technical assistance to community agencies, governmental units, partner programs, individuals, and walk-ins as needed. + Processing incoming referrals, including inquiries from individuals, families, and providers, and tracking progress through to service connection. + Sharing referrals and decision making with the identified individual to receive services through in person assessments and established intake processes. + Facilitating and assigning community agencies selected referrals for individuals who are determined eligible. + Creating, maintaining, and updating a list of community resources; assisting with linkages to those services; and identifying specialty services available in the community for individuals with a Serious Mental Illness (SMI). + Collecting monthly statistical data and reports on referrals and outcomes to SPOA team. + Traveling to local hospitals, residential facilities, client residences, training sites, and/or other community locations. + Performing other duties as assigned. Requirements + BS/BA degree with 2 years of human service and utilization review experience or equivalent education and work experience in behavioral health. (see CCSI's equivalency chart. (https://www.ccsi.org/CCSI/media/pdfs/Equivalency-chart.pdf) ) + Familiarity with OMH guidelines, terminology, and processes. + Computer skills proficient in Word, Excel, and Outlook + Excellent written and verbal communication and interpersonal skills. + Experience working with individuals with disabilities. + Individuals with lived experience in behavioral health systems are encouraged to apply. Additional Requirements + Satisfactory completion of all required background screenings. + Be able to work flexible hours to accommodate meeting schedules. + A valid Class D New York State Driver’s License and access to a motor vehicle insured with $300,000 liability coverage to meet the transportation requirements of the position.
