Neighborhood Living Program has nearly 30 years of experience serving the unsheltered behavioral health (BH) population of Allegheny County. This role involves working as a Residential Service Coordinator (SC) to provide on-site residential support as part of a multidisciplinary team. Clients live in Single Room Occupancy units on the 4th floor of the newly renovated Second Avenue Commons building.

The position involves coordinating mental health services for individuals with serious mental health diagnoses. Key responsibilities include assessment, service planning, coordination, referral, and re-evaluation to ensure community stability in relationships, housing, and employment. This work aligns with the Agency’s vision, mission, values, and mental health Recovery Principles, emphasizing a recovery-oriented approach.

This position is eligible for a sign-on bonus of $3,000 for an 18-month work commitment!

Responsibilities:

 Advocate for individuals to access necessary services. Participate in On-Call Service. Assess strengths and needs through interviews and reviews. Attend training programs for best practices. Ensure monthly contact and provide comprehensive service coordination. Facilitate service planning meetings. Develop and maintain relationships with individuals and their support networks. Ensure needs are met through various supports and services. Evaluate services and review cases with relevant parties. Foster a cooperative agency culture and participate in projects. Promote collaborative service planning. Maintain a catalog of community resources. Keep accurate records of activities. Provide culturally competent services. Coordinate mental health services as part of a treatment team. Serve as the primary resource for individuals and families. Communicate expectations and ensure service coordination. Encourage recovery and community inclusion.
