We are seeking an ambitious and goal-oriented Clinical Pharmacist with over 5 years of experience to join our primary care organization. The successful candidate will play a critical role in improving patient outcomes through effective medication management, patient education, and collaboration with our healthcare team. The role requires strong critical thinking skills and the ability to use clinical judgment to make informed decisions about patient care. Must be able to work Eastern Time Zone hours 8-5pm
Role Specific Responsibilities (executed in person and/or virtual)
Utilize evidence-based medicine and clinical judgment to evaluate the suitability of pharmacotherapy for high-risk patients with chronic conditions, including but not limited to diabetes, hypertension, heart failure and COPDExecute clinical protocols as allowed per organization and state regulatory agenciesOptimization of drug coverage benefits for appropriate care and reduced costs (preferred formulary products, lower cost alternatives and home delivery pharmacy)Collaborate with physicians, nurses, and other healthcare professionals to optimize patient careParticipate in high-risk rounds with a multi-disciplinary team, utilizing highly effective communication skills to discuss patient progress and adjust treatment plans as necessaryExcellent patient counseling skills and the ability to communicate complex medical information to patients and healthcare providersDisease state education for patients on appropriate use of medications and self-managementEngage in quality improvement initiatives aimed at enhancing medication safety and efficacyPost-discharge medication reconciliation for high-risk transition of care patientsDocument all activity within clinic EMR and other required pharmacy or clinical management systemsSupport CMS Stars and HEDIS quality measures for successful clinic performanceMaintain accurate and thorough documentation of clinical interventions and patient care activitiesConduct in-service education and training sessions for healthcare staff as well as develop and disseminate educational materials related to pharmacotherapyStay current with advancements in pharmacotherapy and clinical guidelines.Provide consultation to healthcare providers on complex medication management issuesUse your skills to make an impact

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting.
