Assist clients to complete applications; conduct routine interviews, collect social and financial information, make referrals to appropriate agencies and follow up. Maintain current community resources and eligibility requirements. Provide excellent customer service to clients, specialists and community organizations. The Senior Community Services Assistance engages in collaboration with multiple team members to deliver integrated, person-centered, and comprehensive care to eligible children and their families to successfully address the complexity of chronic health conditions. Effective communication, organization, efficient time management skills, and the ability to successfully multi-task are essential to success in this position. Builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support, and advocacy. Reports to the CSHCS Program Manager. Examples of Duties + Plans, schedules and participates in interviews of program participants to obtain background information, to determine eligibility. + Counsels’ participants on program requirements and services. + Determines the most effective method for providing service to non-routine participants or those with special requirements and explains available options. + Refers participants to other agencies or programs for specialized services. + Follows up on referrals and maintains data on the numbers of clients referred. + Reviews and revises policies or procedures designed to achieve the objectives of the assigned agency. + Performs community outreach to inform the public and other service providers of agency objectives and activities, which may require work outside of normal work hours. + Maintains contacts with public and private social agencies to obtain information referrals. + Attends, participates in a professional meetings and conferences designed to improve and update knowledge and skills related to service delivery. + Answers inquiries from the public and other municipal or regulatory agencies. + Assumes additional duties and responsibilities in the absence of superiors. Minimum Qualifications Qualifications (required): Bachelor’s degree in the social sciences, education, psychology, guidance and counseling or related field. One (1) year of recent experience as a supervisor, preferably in a social, economic or community development program. Three (3) years of experience in case work, group work, individual and group counseling and guidance, social research or community or public relations. Equivalent combinations of education and experience may be substituted to meet the education and experience requirements of this position. KNOWLEDGE OF: + Computer software and hardware. + Diverse groups, working within a multicultural workforce, and sensitivity and appreciation to cultural differences is required. + Factors affecting the health of a community (e.g., equity, income, education, and environment). + Assets and resources that can be used for improving the health of a community (e.g., Boys & Girls Clubs, public libraries, hospitals, faith-based organizations, academic institutions, federal grants, fellowship programs). + Roles of governmental public health, health care, and other partners in improving the health of a community. SKILL IN: + Use of personal computers, common office word processing, spreadsheet and database software. + Oral and written communication.
