VillageCare is looking for a self-motivated and passionate RN with previous Care Management experience for our Senior Care Management Supervisor position. This role works with the Care Manager Supervisors and the Care Managers to assess, plan, implement, coordinate, monitor and evaluate services and outcomes to maximize the health of our members. The Senior Supervisor is responsible for working closely with Directors of Care Management to provide oversight of staff, including; maintaining productivity, identifying training needs, and ensuring all company policies are maintained by staff. \n

Some daily job functions will include: \n
\n\nSupervise and manage the activities of care management staff, ensuring coordination and communication across members of the Interdisciplinary Care Team\nReview and approve care plans and regularly meet with staff on member progress.\nDevelop workflows, policies, and procedures. \nLead the development of care management models to improve the efficiency and effectiveness of care delivery.\nLeverage and drive data for the Care Management Team, to create process improvements.\nCollaborate with other departments cross functionally regarding care management and\/or Health Service initiatives.\nMaintain knowledge of managed care regulations and participate in the Compliance Program.\n\n

We would like to speak to those who bring a Bachelor's degree along with an unrestricted NYS RN license. Seeking those who have 3-5 years practical experience in care management within a managed care setting. Certification in Care Management preferable.\n
