Responsibilities What Will You Be Doing?Community-Based Support: Provide support services for child, adolescent, or adult clients in the community to help them achieve and maintain rehabilitative support.Client Engagement: Meet with clients to review mental health symptoms, assess needs, and provide interventions as outlined in the treatment plan.Assessment and Planning: Review all assessments and notes related to the individual's history, diagnosis, and treatment plan. Assist clients and their families in managing or preventing crisis situations.Goal Facilitation: Support clients in articulating and achieving their goals by identifying strengths and providing skills support.Skills Development: Teach coping skills, link individuals to community resources for basic needs, and develop basic living skills to prevent relapse and ensure long-term recovery.Crisis Management: Assist in the development of safety plans for clients to utilize during mental health crises, identifying crisis numbers and natural supports.Resource Coordination: Ensure clients have access to necessary rehabilitative, medical, social, and other services and supports. Assist in identifying resources for families in need and direct families in crisis to appropriate staff for intervention.Collaborative Support: Work in partnership with case therapists to help clients achieve their potential. Attend community meetings to enhance relationships with community partners and ensure continuity of care.Service Utilization Tracking: Monitor and track the utilization of services. Review and update treatment plans as needed, and support clients in making appointments and attending meetings to reauthorize or update treatment plans.Record Keeping: Update contact information in the electronic medical record (EMR) to ensure other mental health providers can maintain contact with the client.Staff Collaboration: Attend staff meetings for continuing education, policy updates, and case staffing. Report any concerns regarding clients' mental health symptoms, basic needs, compliance with treatment services, or changes in insurance status to the Program Director, Clinical Supervisor, and/or medical staff. Successful staff exude a passion for serving vulnerable individuals and families. A flexible schedule is important for this role and allows you to have control over your session planning. Ability to work independently and manage time well. Ability to collaborate with a team to accomplish goals.Computer proficiency to include ability to navigate online electronic medical record system.Qualifications:
