Job Summary

\n

The Transplant Coordinator RN performs patient care coordination tasks in our healthcare team. They assess patients, develop personalized plans, and gauge responses.

\n

The Transplant Coordinator helps patients navigate healthcare smoothly by understanding needs and guiding them to avoid issues. They also assign tasks to staff according to law, patient health, and staff skills. This ensures top-notch care for all patients.

\n

What You'll Do

\n\nWorking with various medical professionals to assess transplant candidates.\nTasks include interviewing families and patients, reviewing results, and partnering with physicians for optimal care.\nCoordinating testing for waitlist candidates and aligning care with doctor's instructions.\nMonitor test results for issues.\nStay in contact with patient and healthcare team.\nYou will manage aftercare for transplant recipients by coordinating admissions, tests, visits, and imaging.\nPartner with primary and referring physicians to monitor complications and abnormal test results.\nYou will create educational programs for candidates and recipients. These cover evaluation, allocation, care, impacts, and prescription management.\n\n

What Will Make You Successful In The Role

\n\nA deep knowledge of nursing and patient care standards and procedures.\nKnowledge of laws, regulations, standards, guidelines from certifying bodies. Familiarity with nursing care standards, protocols, policies in our department.\nUnderstanding medical terms is essential. Also important are practices like health promotion, risk reduction, and disease prevention.\nKnowledge of medications and their effects is crucial.\nCommitment to professional nursing ethics and patient privacy rights.\nExcellent communication skills, both verbal and written.\nExceptional social skills to engage effectively with a wide range of people.\nCapacity for critical thinking and problem-solving.\nAbility to monitor patient conditions and communicate changes to nurses and doctors effectively.\nAbility to give quality care. Must meet patient needs. Planning, evaluating, and adjusting care plan as needed.\nBasic computer skills, which should include Microsoft Office, information security, scheduling and payroll systems, electronic medical documentation, and email.\n\n

LOCATION:  Baylor University Medical Center, Dallas, Texas

\n

SPECIALTY/DEPARTMENT:  Heart and Lung Clinic

\n

SHIFT/SCHEDULE:  Monday-Friday, PRN days, 8-hour/day, with flexibility  Two minimum shifts required per month; periodically could up to 16-24 hours a week.

\n

Qualifications

\n\nAssociate's Degree.\nPrevious work experience of at least 2 years in a similar role.\nRegistered Nurse.\nBasic Life Support (BLS), or the capability to gain BLS within 30 days of being hired.\n
