**Job Summary and Qualifications**
The Stroke Program Coordinator position involves the coordination of clinical management of the stroke patient population across the entire continuum of care. Serves as a leader for the Stroke program performance and improvement initiative as well as monitors outcomes. Maintains clinical expertise and acts as a clinical resource/role model for all areas caring for stroke patients. Provides and/or coordinates educational activities with nursing management and the education department for physicians, staff, patients, EMS and referral hospitals. Acts as a community liaison on behalf of the program and hospital. Directly involved in the marketing/promotion of services provided. This role provides education and leadership, serves as a resource and guide to identify current concepts and evaluates their usefulness in improving outcomes, quality and patient safety. Assists in the coordination of reports and analysis of data. Ensures and models a commitment to the organization’s vision, mission, and values to support clinical outcomes. Supports the promotion of a patient safety culture; emphasizing risk reduction while analyzing trends in events to support process improvement efforts that align with the high reliability strategies. Maintain communication with administration, medical staff, and departmental leaders regarding process improvement activities.
**What you will do in this role:**
+ Maintains and demonstrates clinical expertise in the area of stroke. Remains an expert resource for adherence to treatment in relation to clinical trials and clinical practice guidelines and provides resources to physician and staff.
+ Completes at least 8 hours stroke continuing education (CNE) annually.
+ Reviews current stroke clinical practice guidelines, policies and procedures annually and facilitates requisite revisions. Ensures facility adopts the most current clinical practice guidelines.
+ Collaboratively works with physicians and interdisciplinary teams to develop, implement and annually revise pathways, treatment algorithms, order sets and processes including stroke codes through entire facility.
+ Oversees universal usage of order sets and processes for the stroke program. Identifies variances and facilitates variance resolution.
+ Ensures Stroke Performance Measures are received by the Joint Commission, Stroke Get with the Guidelines and Louisiana Emergency Response Network per the required timeframes.
+ Responds as part of the response team to in-house and ED stroke codes to facilitate the nursing evaluation, administration of thrombolytic and rapid transport of large vessel occlusion stroke patients.
+ Coordinates with therapists, social workers, and case managers to ensure patient flow occurs expeditiously and efficiently.
+ Identifies opportunities for improvement by incorporation of data and patient outcomes in continuous quality improvement processes by developing and implementing action plans.
+ Collaborates with nursing leadership in order to develop and promote stroke nursing standards of care.
+ Directs development and implementation of post-discharge stroke patient perception of care.
+ Participates in Louisiana Emergency Response Network stroke initiatives. Attends required committee meetings to maintain State of Louisiana Stroke Designation.
+ Facilitates development of stroke physician/staff orientation program, annual competencies and specific educational programs for entire range of healthcare professionals
+ Collaborates with and educates EMS/Referral hospitals to ensure delivery of quality care and rapid access to Rapides Reginal Medical Center.
+ Leads the multidisciplinary stroke team and committee meetings.
**What qualifications you will need:**
+ Advanced Cardiac Life Support must be obtained within 30 days of employment start date
+ Registered Nurse
+ Bachelors Degree
+ Basic Cardiac Life Support, or CPR for Healthcare Providers must be obtained within 30 days of employment start date
+ A minimum of 3 years experience in an acute care setting **_Required_**
