**Job Summary**
**This position will support case management outcomes for The Heart Hospital Region: Denton, Dallas, Fort Worth, McKinney, Plano, Waxahachie and Corinth.**
The Program Outcomes Manager ensures patient care continuity by partnering with medical teams before discharge. Responsibilities include working with doctors, nurses, and other healthcare team members. They advocate for families, coordinate care strategies, and improve understanding between families and staff. Contributions enhance care quality and prioritize a family-centered approach.
**Essential Functions of the Role**
+ Work with healthcare professionals, involve families in treatment, and educate them. Linking different departments and professionals.
+ Serving as patient families' contact.
+ Holding meetings to address concerns, educate, identify resources, and discuss post-treatment care.
+ Conducting post-discharge patient calls to confirm the family's understanding of discharge instructions and future appointments.
+ Identifying ways to improve treatments, services, discharge times, and overall patient needs during treatment and recovery.
+ Organizing and coordinating case conferences for staff and providers to maintain and enhance care quality.
+ Participating in continuous improvement projects to enhance patient services and healthcare. Responsibilities involve examining outcome measures to refine patient discharge practices.
+ Recognizing and fulfilling the training needs of staff in areas such as family-centered care and family discharge preparation.
+ Collaborating with the executive team for successful execution of projects and educational efforts.

+ Possessing top-notch oral and written communication skills is a must.
+ The capacity to track, evaluate, and document a patient's development in relation to a plan of care is pivotal.
+ The capability to aid patients in accessing community resources is key.
+ Working well with medical staff internally and externally is crucial for our organization's success.
+ Collaboration with partners is necessary.
+ An effective adjustment and composed response system to ever-changing and crisis scenarios is critical.
+ The ability to maintain the confidentiality of all patient-related information in line with all relevant policies and regulations is indispensable.

**LOCATION:** The Heart Hospital Baylor, Plano, Texas
**SPECIALTY/DEPARTMENT:** Cardiology
**SHIFT/SCHEDULE:** FT Days, M-F; Hybrid with 3-4 days remote depending on meeting schedule at Plano.
**Qualifications**
+ Possession of a Bachelor's degree in Nursing
+ A minimum of (3) three years' relevant professional experience
+ Registration as a Nurse. (RN)
