RN Program Coordinator - Sleep Medicine
Job Summary The RN Program Coordinator functions as an RN member of the Sleep Healthcare team in a manner consistent with their scope of practice in Pennsylvania and in concert with policies and procedures. The Program Coordinator is accountable for the effective management, coordination of care and development of education for specific patient populations in our Sleep Center. The coordinator partners with the Sleep Medicine providers, sleep lab staff and others to coordinate patient care and provide clinical support in clinic and by phone. ESSENTIAL FUNCTIONS Conduct interviews with patient, family and other health care agencies to obtain past medical records, social and medical history and to assess their understanding of the plan of care. Prepare and analyze pre-procedure clinical data and consult with multidisciplinary team. Educates patient and family regarding all aspects of disease management and diagnostic procedures. Develop nursing care plans to ensure standards of care are achieved. Proactively review patient schedules to ensure efficiency in clinical workflows, identify and resolve potential conflicts. Communicate and problem solve with multidisciplinary team to solve complex patient care issues. Ensure appropriate resources are available to support patient care and procedures. Complete prior authorizations for scheduled testing/ procedures, and complete prior authorizations for patients Assess patient’s benefit coverage and communicate with hospital personnel responsible for payment of procedures, treatments and issues of medical necessity. Educate patient and family of signs and symptoms that require a follow up telephone call or triage visit. Conduct and document patient consultation to problem solve with the multidisciplinary team ensuring quality follow up medical care and interventions are provided. Collaborate with medical providers, nursing personnel and other health care agencies to facilitate patient and family needs. Retrieve Sleep Clinic phone messages and complete the follow up actions. Coordinates/accelerates office visits/ PSG Studies in collaboration with the providers and sleep team Review any parent/patient feedback reported in the sleep lab in conjunction with the sleep study lead tech/office manager. Assist with coordination of Sleep Lab and clinic schedule for rotating fellows. Call families with Sleep Study results. Send copies of study reports to the appropriate physician. Provides phone follow up for patients on home therapies when appropriate. Develops/maintains clinical database in collaboration with the Sleep Lab Lead Tech and the providers. Attend regular Sleep Lab/Program meetings. Complete all duties as a clinic nurse during sleep clinic. Complete per to peer reviews with insurance as needed. Review and reconcile scheduling, billing, referrals, and other patient flow needs. All other duties as assigned. #STC Qualifications EDUCATION REQUIREMENTS Completes and maintains all competencies and trainings as required for role.
