The RN in this position is responsible for the care coordination of the structural heart program. This RN will work in tandem with the current structural heart coordinator. This position will require two days one week and three days of work the second week of the pay period. Although this position is Monday through Friday, no weekend, no holidays, there is always the potential for a last-minute add-on structural procedure that would require after-hours or weekend coordination.
Responsibilities
The Coordinator helps to drive a quality based, patient-focused program. A coordinator is devoted to managing the program implementation and execution, patient /physician notification components, education, and outreach. The Coordinator is the primary advocate of the program, both internally and publicly.Collects and analyzes potential patient data from various channels, including echo alert data (inpatient & outpatient), cath lab data, hospital and ambulatory medical records.Develops structured communication pathways to ensure identified patients’ data is communicated in a timely manner.Offers assistance with referral coordination for patient who meet established criteria.Provides follow up to each case referred to determine appropriateness of referral, timeliness of disposition, and customer/patient satisfaction, and takes immediate action to problem-solve whenever necessary.Coordinates data collection for clinician decision making related to this patient population.Maintains, manages and reviews data in valve tracking data base.Assist in developing and revising educational materials for the referral base related to these disease processes.Provides education to individuals within the care continuum of patients with cardiothoracic surgical needs and structural heart disease.Receive and review patient referrals for appropriateness, reviewing with an attending provider as neededEstablish contact with patient/family. Explain process and work-up.Perform functional assessments and frailty testingHave all testing available for review by cardiologist and CTS at clinic visitParticipate in development of treatment planPatient and family educationCoordinate any additional testing i.e. cath, TEE, CTUpload required data to appropriate vendor and maintain communication with device representativesFacilitate scheduling and authorization of structural heart proceduresParticipates follow-up visits and coordinates further follow-up per registry requirement.Coordinates concerns among multiple departments including but not limited to OR, cath lab, anesthesia, CT, echo, vascular, hospital units.Initiates and coordinates outreach programs.Tracks required registry data to be sure requirements are being met.Attends weekly valve conference.Establish strong working relationship with surgeons, interventionalist, advanced practice providers, and device representativesFlexibility to adapt to new technologies and procedures as they evolve: TAVR, MitraClip, WATCHMAN, etc.Experience/attributes

Facilitates the development, implementation and evaluation of program services and initiatives.Plans, implements, evaluates, and coordinates appropriate patient care.Maintains program documentation and review to support regulatory and compliance regulations.Assists with determining appropriate training for the staff and developing the learning program.Collaborates with advanced practice providers to ensure quality outcomes.Collaborates with others to assist with problem identification and resolution, cost containment issues, implementation of new services, and systems/performance measures.May participate in injury prevention initiatives and/or activities.Works in a constant state of alertness and safe manner.Performs other duties as assigned.

Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.Frequent sitting, standing, walking, reaching and repetitive foot/leg and hand/arm movements.Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.Frequent use of hearing and speech to share information through oral communication. Ability to hear alarms, malfunctioning machinery, etc.Frequent keyboard use/data entry.Occasional bending, stooping, kneeling, squatting, twisting and gripping.Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.Rare climbing.REQUIRED PROFESSIONAL LICENSE AND/OR CERTIFICATIONS
Resuscitation Certification Requirement: Basic Life Support Healthcare Provider (BLS HCP) required within 90 day of employment. Other resuscitation certifications may be required dependent on department.
