ArchCare Program: PACE
Overview:
ArchCare cares for people of all ages and faiths where they are most comfortable and best able to receive it – at home, in the community and in nursing homes. As the Continuing Care Community of the Archdiocese of New York, we see enhancing the lives of our elders and others as more than just a job. To us, it’s a privilege and our calling.
ArchCare Senior Life (PACE) is a long-term Medicaid/Medicare plan designed specifically for seniors who need nursing home level care but want to stay at home. With Senior Life, participants have a dedicated Care Team to coordinate everything, so they are able to live comfortably where they are, knowing they’ll get the coordinated care they need. PACE provides and coordinates care at our Wellness Centers, in participant’s homes and in other settings.

Assesses patient’s physical and psychosocial needs.Completes assessment documents including UAS, problem lists and interventions and other required documentation; documents changes in the patient’s condition and details care provided by completing all required PACE documentationDevelops and implements the home care components of PACE (personal care and home health services), including family and other caregivers, and works with colleagues to develop a comprehensive Interdisciplinary plan of careProvides initial home care assessments of new patients, and reassesses at appropriate intervalsCoordinates the medication delivery and administration processesEducates and assesses the patient/family understanding of nursing needs and their capabilities for self-care or family completion of allowable nursing tasks.
