GENERAL SUMMARY

Transplant Coordinators coordinate all aspects of care for both transplant recipients and donors. A coordinator participates in medical, administrative, research and teaching duties. The coordinator is an active member of the multi-disciplinary transplant team. Development and implementation of IPD critical Pathways IPD discharge planning Facilitating and managing the use of home care Triage of OPD and IPD medical issues with referral to appropriate health care professionals Communications with insurance companies and referring physicians Quality assurance projects Promoting health maintenance education and follow-up Donor & Recipient Family and patient management 

PRINCIPLE DUTIES AND RESPONSIBILITIES

 Clerical: Gain information related to diagnosis, history, review of medical records for evaluation of recipients and donors Obtain records from other facilities, patients, families and other health care providers. Communicates with the insurance companies and referring physicians to ensure payment/coverage. Clinical: Coordination of care for admitted patients, assessment, writing progress notes, completing flow sheets, communicating plan of care to the patient. Management and staffing of OPD Transplant Clinics. Clinical assessment of patients and donors, pre and post-transplant. Review medications and side effects for IPD and OPD patients. Develop individualized plans of care for each patient. Collecting and analyzing test results. Provides psychosocial support and referrals to agencies for assistance. Provide patient teaching of health care maintenance, disease process, transplant process, medications, donor awareness, pre- and post-operative teaching, testing procedures, donor procedures. For those Coordinators working with advanced heart failure patients, VAD patients and heart transplant patients throughout the system, the additional responsibilities are as follows: Prepares machinery for device implantation including sterilization prior to surgery. Initiation of VAD flow during the operation and monitoring of the hemodynamic performance during and after the operations on all elective and emergent operations involving Vads. Serves as the primary on-call emergency contact to family members and all healthcare delivery entities (i.e., EMS, hospitals, emergency departments, subacute care facilities, etc.) for purposes of equipment maintenance/malfunctioning, hemodynamic issues and patient care triage for patients with VADS. Participates in an on-call rotation requiring less than 45 minutes travel time to the hospital and often times requires on-site presence when VAD or heart transplant patients are transported to HFH. Maintains appropriate data bases as required. Other: Provides continuing education for promotion of donor awareness as well as chronic illness education to hospital and community medical staff. Coordination of organ donors, between organ procurement agency, transportation, surgeons, recipient coordinators and patients, assists in matching of recipient to donor. Participates in data collection for UNOS, national registries, and for departmental and FDA research projects. Develop, monitor and implement clinical protocols for patient care, IPD and OPD. Maintain lists of listed recipients for the cadaver waiting lists Provide and participate in orientation of medical and hospital personnel with regard to the functions of the transplant service. Develop, collect and analyze data concerning quality assurance projects.

EDUCATION/EXPERIENCE REQUIRED:

Bachelor's degree in nursing preferred. Organizational and Interpersonal skills to effectively communicate with patient care personnel and relate to the patients and their families. Minimum of two (2) years clinical nursing experience in an inpatient or outpatient setting. Analytical ability and motivation necessary to effectively plan, deliver and implement plans of care to in acute and outpatient situations. For those working as a Coordinator of Heart Transplant/with VADS: Experience in ICU or OR preferred.

 

 

CERTIFICATIONS/LICENSURES REQUIRED:

 

Current RN license in the State of Michigan.
