Unlock your potential at Rapides Regional Medical Center! **Job Summary and Qualifications** This position is responsible for improving the overall quality and completeness of clinical documentation by facilitating modifications to clinical documentation through extensive interaction with physicians, nursing staff, other patient care-givers and medical records coding staff. It ensures that appropriate reimbursement is received for the level of services rendered to patients with a DRG based payer. It also ensures the accuracy and completeness of clinical information used for measuring and reporting physician and facility outcomes. **What you will do in this role:** + Obtains and promotes appropriate clinical documentation through extensive interaction with physicians, nursing staff, other patient caregivers, and coding staff to ensure that the documentation of the level of service rendered to the patient and the patient’s clinical complexity is complete and accurate. + Reviews medical records and identifies potential gaps in clinical documentation for specified patient types and payer populations as directed on admission and throughout hospitalization. + Queries physicians and other caregivers as necessary via approved written communication mechanisms to obtain accurate and complete documentation that supports the severity of patient illness and risk of mortality. + Works closely with coding staff to assure documentation of discharge diagnoses and any co-existing comorbidities are a complete reflection of the patient’s clinical status and care. + Demonstrates basic knowledge of coding standards and application to ongoing evaluation of medical record documentation. + Consistently meets established productivity targets for record review. + Educates all internal customers on clinical documentation opportunities, coding and reimbursement issues. + Conducts follow-up reviews of clinical documentation to ensure points of clarification have been recorded in the patient’s chart. + Follows departmental guidelines for clocking in and out on the time clock. + Performs other duties as assigned. **What qualifications you will need:** + Registered Nurse + Registered Nurse Diploma, or Associate Degree + Utilization Review and coding experience preferred + Demonstrated ability to develop strong relationships with physicians and peers + Requires demonstrated leadership skills in order to encourage a spirit of mutual cooperation + Requires a high level of interpersonal communication skills + Working knowledge of personal computer skills in standard Office software applications Rapides Regional Medical Center is a 350+ bed hospital and ER in Alexandria, Louisiana.
