What you will do as an RN Care Coordinator:
Coordinates care across the health care continuum within designated sub-specialty population(s)Serves as a single point of contact by facilitating and coordinating transitions in care within and between care settings, providers, and servicesCompletes pre-visit planning and post-visit wrap-up and determines vital follow-up appointments and/or treatments to optimize patient visitsFacilitates proactive disease management and preventative health care using evidence-based practiceRooms patients for the surgeon(s) as needed, complete clinic readiness including records review and provides administrative support as needed.What you will need:
Current Minnesota RN licenseBLS certificationDemonstrated experience in patient advocacy and education, coaching, and proactive health care management along with EMR/digital literacyPreferred Qualifications:
GI experience preferredPotential to assist and/or perform esophageal manometry, hydrogen breath testing, 24-hour ph, and pill cam.Location: 909 Fulton St SE, Minneapolis, MN 55455
Hours: 0.8 FTE 32 hours a week between Monday-Friday clinic hours
