We are seeking a Registered Nurse – Care Coordination Oncology Navigator with our Care Coordination Team.
The RN Care Coordination Navigator ensures timely access and seamless coordination of services throughout the continuum of care for a designated patient population which includes cancer patients receiving chemotherapy concurrently or consecutively; patients requiring referral to a tertiary care provider; and patients with complex care or women’s health issues. The RN Care Coordination Navigator functions as an active health advisor, educator and advocate throughout the process of screening, diagnosis, and treatment of cancer or other conditions. This position may be responsible for facilitation of scheduling and coordination of follow-up care if necessary. This position shares knowledge with patients and families regarding internal and external support services and resources, and tracks data on patients to ensure completion of recommended treatment plan and follow-up care. The RN Care Coordination Navigator works directly with the health care team to deliver innovative health management practices to the patients and community of Aspirus.
HOURS: Full-time; 1.0 FTE or 80 hours per two-week pay period. Day Shift.
Experience/Qualifications
Knowledge of oncology medical and nursing clinical care normally acquired throughreceipt of a Bachelor of Science Degree in Nursing. Master’s Degree in Nursing or closely related field is desired.Possesses an active Michigan or Wisconsin RN nursing license.Certification in Oncology required.Requires previous experience working with medical and radiation oncology programs.Navigator Certification preferred.Current CPR certification or within 90 days of hire.Current ACLS certification or obtained within 90 days of hire.Two or more years’ clinical experience in women’s health.Skills in organizing and prioritizing work along with multi-tasking simultaneously.Skills in working effectively as a dedicated, creative and resourceful team player.Possesses excellent oral and written communication skills as well as excellent interpersonal skills.Demonstrates sound clinical judgment, decision-making, and problem-solving skills.Willingness to be a change agent and positive role model.Knowledge of motivational communication techniques to engage patients/families.Ability to demonstrate a strong, autonomous work ethicwith qualities of personal and professional integrity.Detail and goal oriented demonstrated by personal initiative and accountability.Possesses confidence to communicate and outreach to other community health care organizations and personnel.Employee Benefits
